Relação entre toreoidite auto-imune e carcinoma papilar da tireóide

Resumo

Objetivo

Verificar a relação entre a tireoidite auto-imune e o carcinoma papilar da tireóide em pacientes submetidos à tireoidectomias entre 1994 e 2003.

Métodos

Foram selecionados 89 pacientes com carcinoma papilar pareados com 89 pacientes portadores de bócio colóide integrantes do grupo-controle, randomizados segundo o critério de idade e sexo. Todos os pacientes foram avaliados quanto à associação com tireoidite auto-imune.

Resultados

Dos 89 pacientes com carcinoma papilar, 17 (19,1 %) apresentavam tireoidite auto-imune, enquanto nos 89 indivíduos do grupo-controle apenas 4 (4,5%) cursaram com a doença inflamatória, evidenciando uma diferença significativa na freqüência de tireoidite nos casos de câncer papilar (x2= 9,12; p= 0,003).

Conclusão

Os resultados do presente estudo demonstram uma ocorrência maior de doença inflamatória em portadores de carcinoma papilar da tireóide quando comparados com portadores de bócio colóide. Essa relação indica que novos estudos devem ser realizados para verificar se os mesmos estímulos que contribuem à tireoidite auto-imune podem agir como iniciadores do câncer da tireóide.
